Mail in Registration*

For mail-in registrations, download registration application here. Print one for each participant.

1. A registration form for each participant must be filled in including any for riders under the age of 18.
2. The waiver must be signed on the registration form. All waivers for children under 18 must be signed by parent or guardian. You will need to download, print and sign the waiver/release and bring it with you to the Ride at check in.
3. A check or money order for the registration fee(s) must accompany the registration form(s), as well as the $100 fundraising pledge donation. See the Donations page for more information on the $100 fundraising pledge.
4. Mail registration form(s) in along with a check made payable to:

Registration Fees

Adults*
The registration fee is $25 until February 12, 2010 and $30 after February 12, 2010.

Children (12 years and younger)*
The registration fee is $10 until February 12, 2010 and $15 after February 12, 2010. Child t-shirt only available in Adult Small.

Day of Event Registration (non-refundable)

Adults*
The registration fee is $30.

Children (12 years and younger)*
The registration fee is $15. Child t-shirt only available in Adult Small.

*Minimum pledge to participate is $100 fundraising for each rider 18 years old and up.
*Family pledge $250 per family of riders.
*Registration fee does not count toward fundraising pledge.
*Team fundraising minimum is determined by the number of members. $100 per member.

Be a Team Captain*

Be a team captain by forming a team of two riders or more by registering for the ride at:
http://www.active.com/donate/ridewithme2010, click on “Become a fundraiser” then click “create a team”.

Waiver/Release Form
